74 PMC Primary Care Practices (95%) Receive Blue Cross Blue Shield of Michigan’s PCMH Designation

The Practices are Members of PMC, a Physician Organization Managed by Medical Advantage Group

FLINT, Michigan, September 1, 2020 – Primary care practices in the Medical Advantage managed physician organization Professional Medical Corporation (PMC), recently received 74 Blue Cross Blue Shield of Michigan (BCBSM) Patient-Centered Medical Home (PCMH) designations. BCBSM’s PCMH program is a comprehensive approach that creates care teams that work with patients to keep them healthy while monitoring their care on an ongoing basis. The PCMH program uses registries to track and manage conditions like asthma and diabetes.

PMC consists of over 460 independent primary care and specialty physicians based throughout the Genesee, Lapeer, Shiawassee and Monroe areas. The physician organization offers member practices access to pay-for-performance programs, health plan contracting, technology, process improvement and performance improvement resources.

Medical Advantage’s practice transformation team works with PMC practices to help them achieve PCMH status. The team works on-site with practice staff to implement various patient-centered processes to qualify for PCMH designation. In 2020, Medical Advantage helped nearly 200 primary care physician practice clients achieve designation. Since, 2009, Medical Advantage has assisted in over 1,000 practice transformations.
